Job Summary

This is a dynamic and challenging role that requires a skilled and compassionate nurse to provide high-quality care to our residents. As an On-Call Care Specialist, you will be responsible for delivering personalized care and support to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Provide Medication Administration and Medical Treatment: Administer prescribed medications, provide medical treatment, and perform personal care services to residents in Assisted Living and/or Memory Care.
  • Assess and Respond to Residents' Changing Health and Condition: Monitor, assess, and respond to residents' changing health and condition, coordinating and evaluating medical needs and/or activities of daily living to formulate and provide a safe, balanced, and adequate program of care.
  • Develop and Implement Personal Service Plans: Participate in the initial and ongoing development of the Personal Service Plan, including Focus areas and Interventions, and communicate updates and/or coordination of care to other associates, outside health services, responsible parties, and physician.
  • Provide Education and Support: Provide educational in-services to enhance associates' knowledge and for better quality of service to residents, and maintain familiarity and comply with Company Policies and Procedures and State Regulations.
  • Manage and Coordinate Resident and Family Requests: Manage and coordinate assisted living or memory care resident and family requests and/or complaints in a professional and timely manner, with sensitivity and a positive attitude.
Requirements
  • Education and Experience: Graduation from an approved vocational nursing or registered nursing program, and California License Vocational Nurse (LVN) and/or California Registered Nurse License (RN).
  • Certifications and Clearances: Current First Aid Certification, DOJ and FBI fingerprint clearance, and health screening and negative TB test.
  • Skills and Abilities: Ability to communicate clearly and effectively, interact well with residents, family members, and staff, and demonstrate adequate time management skills.
