Job Overview

Under the guidance of the Director of Crisis Services, the Hospital Liaison plays a crucial role in executing the DWIHN Utilization Management Plan within the Crisis Environment. This position is responsible for conducting informal evaluations, planning, advocacy, coordination, and monitoring to support individuals facing a behavioral health crisis in accessing necessary crisis services in a medically appropriate and minimally restrictive setting.

KEY RESPONSIBILITIES:

  • Conducts informal evaluations, planning, advocacy, coordination, and monitoring to assist individuals in crisis in accessing appropriate services.
  • Utilizes clinical data to aid community partners in crisis placements.
  • Facilitates clinical discussions between placement facilities and DWIHN.
  • Provides technical guidance and oversight to crisis intervention service providers.
  • Supports crisis intervention teams in ensuring consumers are placed in suitable environments.
  • Offers consultation on cases to minimize recidivism in inpatient admissions and emergency room visits.
  • Delivers short-term, solution-focused crisis counseling to individuals and families as needed.
  • Establishes and nurtures collaborative relationships with colleagues, hospital personnel, and community partners.
  • Monitors census data for crisis residential services, emergency departments, and inpatient admissions.
  • Consults with hospitals and crisis residential staff to gather comprehensive clinical insights on patients in crisis.
  • Assists in identifying community resources to facilitate timely discharges.
  • Ensures that individuals in crisis awaiting placement can secure appropriate accommodations.
  • Conducts concurrent reviews for consumers.
  • Coordinates with various systems and providers to meet member needs throughout the utilization process.
  • Facilitates discharge planning and ensures individuals are connected with a designated Clinically Responsible Service Provider (CRSP).
  • Coordinates care levels based on discharge disposition, including inpatient placement and outpatient services.
  • Gathers necessary information to serve assigned individuals effectively.
  • Communicates expectations to ensure service providers have all relevant information for effective care delivery.
  • Monitors health changes and takes appropriate actions for consumers.
  • Facilitates meetings with crisis screeners, emergency room staff, and psychiatric inpatient teams.
  • Completes special projects as assigned.
  • Researches and develops policies to enhance DWIHN's crisis continuum.
  • Applies medical necessity criteria to determine appropriate levels of care.
  • Utilizes motivational interviewing and other engagement techniques.
  • Assists consumers in accessing services tailored to their medical, psychiatric, and social support needs.
  • Coordinates discharge planning effectively.
  • Performs additional duties as required.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Familiarity with DWIHN policies and community resources.
  • Understanding of the Michigan Mental Health Code and relevant regulations.
  • Knowledge of clinical care processes, including assessment and treatment planning.
  • Proficiency in behavioral health practices and terminology.
  • Awareness of compliance standards and medical necessity criteria.
  • Skills in assessment, conflict resolution, and effective communication.
  • Ability to work with diverse populations and maintain professional relationships.
  • Strong organizational and multitasking capabilities.

EDUCATION:

A Master's Degree in Social Work, Psychology, Counseling, Nursing, Human Services, or a related field is required.

EXPERIENCE:

A minimum of five (5) years of clinical experience in a psychiatric or behavioral health environment, along with at least three (3) years of professional experience in behavioral health utilization review or management within a community mental health setting.

LICENSE:

A valid State of Michigan clinical license (RN, LMSW, LMHC, LPC, LLP, or PhD) is required. A Limited License may be accepted for this position.

WORKING CONDITIONS:

This role involves work primarily in hospital settings and requires local travel to various facilities. Candidates should be prepared to work a flexible schedule.
