The Youth and Veteran Support Coordinator will identify psychosocial obstacles to career advancement and deliver customized assistance to our young adult and veteran participants to tackle recognized challenges through: a) offering confidential and empathetic individual counseling that directly addresses the spectrum of participants' psychosocial requirements b) pinpointing and directing participants to suitable community resources c) collaborating with staff to devise effective strategies that will enhance optimal learning outcomes for each participant d) evaluating and analyzing the program's service delivery framework to guarantee that participants receive essential support and resources. The primary objectives are to empower participants to effectively navigate personal difficulties, facilitating their transition into employment and fostering their pursuit of growth and ongoing education. This role necessitates close interaction with participants, fostering an atmosphere of trust and support, and engaging with them consistently in ways that contribute to both their success and that of the organization. The Youth and Veteran Support Coordinator will provide assistance during stressful times, alleviate crisis situations, offer counseling, and connect participants to necessary resources.

KEY RESPONSIBILITIES

Participant Development/Case Management/Assessment and Evaluation/Counseling

Conducts interviews with young adults and veterans to fulfill various tasks including but not limited to evaluation, service planning, assessment, incident investigation, and transition planning.

  • Facilitates services to minimize care fragmentation, reduce barriers, and connect participants with appropriate services to ensure comprehensive, continuous access to essential medical, social, educational, and other services tailored to the needs of young adults.
  • Develops individualized action/service plans for assigned cases in collaboration with the service planning team and acts as the team leader to coordinate meetings, assign tasks, and provide guidance and direction. Continuously evaluates and reviews each participant's care plan.
  • Conducts community outreach for supportive services and additional resources for young adults and veterans.
  • Researches new resources for the program (childcare, mental health, emergency housing, etc.).
  • Handles tasks related to client referrals, intake, and placement, assisting with the transition of young adults to ensure their success during and after the program. This includes meeting with participants and staff to complete eligibility determinations and other assessment activities; coordinating with referring agencies and organizational staff; completing and documenting referral information.
  • Completes necessary tasks upon finalization of action/service plans for participants, which include providing post-placement supervision and forwarding records and reports to relevant regulatory agencies in accordance with licensing requirements and applicable laws.
  • Manages all assigned cases and maintains regular face-to-face contact with participants as defined by specific case needs.
  • Educates and counsels participants by fostering a supportive relationship to promote the service plan, including placement and discharge.
  • Engages in recruitment, certification, and evaluation of learning and work environments for participants to ensure they are prepared to be productive. This includes conducting information sessions with staff, writing home studies, periodic evaluations, and maintaining consistent contact with participants and staff team members.
  • Participates in crisis intervention and provides necessary support, including interventions to prevent service disruptions. Engages in on-call rotation as needed.
  • Offers supervision, support, and assistance to participants through regular meetings, phone contact, and email as assigned, continuously evaluating and monitoring participants in learning/work environments.
  • Completes periodic progress reviews and documentation as required by organizational procedures and licensing standards. Collaborates with referring organizations to provide necessary reports and documentation, attending meetings as requested.
  • Maintains accurate, complete, and up-to-date files on all assigned participants.
  • Participates in the record review process and promptly addresses file deficiencies.
  • Conducts initial assessments for each participant.
  • Facilitates regular one-on-one meetings (weekly) with participants, particularly those requiring additional support.
  • Provides life coaching and assesses the intake process.
  • Records relevant information using Salesforce.
  • Oversees the orientation process.
  • Delivers personal development workshops.
  • Plays a significant role in graduation planning and execution.
  • Contacts resources to present in workshops.
  • Communicates updated information with NPower staff.
EDUCATIONAL REQUIREMENTS

At minimum, one of the following must be met:
  • A four-year college degree in Social Work, Sociology, or Psychology from an accredited institution.
  • A minimum of three years of experience in Case Management, Social Work, Psychology, or Counseling.
  • Or at least three years of experience in Human Development.
  • A Master's degree (MSW) in Social Work/Psychology or Counseling from an accredited institution.
  • A second-year graduate student with at least one year of fieldwork.
  • Licensure as a Social Worker/Psychologist is a plus.
POSITION QUALIFICATIONS:

SKILLS & ABILITIES
  • Effective written, oral, and interpersonal communication skills.
  • Ability to maintain confidentiality and demonstrate mature judgment and emotional stability.
  • Proficient in computer usage, comfortable with information management, and capable of prioritizing tasks.
  • Proficiency in utilizing technology to deliver career services.
  • Experience in teaching, presenting workshops, and/or providing counseling in a young adult program setting is highly desirable.
  • A passion for helping individuals grow, develop, and improve their lives and communities.
  • Knowledge of community resources.
  • Experience working with the young adult population.
  • Experience in one or more of the following fields: workforce development, human potential, case management, human resources, or social services.
