The Advanced Practice Nurse in Palliative Care is responsible for facilitating advanced illness management and palliative care for Curo patients, ensuring a thorough, personalized treatment strategy while showcasing outstanding clinical expertise and ethical judgment.

This role involves diagnosis, treatment, consultation, and follow-up under the guidance of the Palliative Medical Director or Primary Care physician.

Key Responsibilities:

  • Collaborate with the Professional Services Corporation (PSC) Physician or supervising physician, Primary Care Physician, and other members of the palliative care team to provide advanced illness management and palliative care through diagnosis, treatment, and follow-up for referred patients.
  • Assume responsibility for advanced illness management from initial assessment to discharge.

Assessment should include:

disease-specific changes, functional decline, nutritional and cognitive status, inadequately controlled pain or distressing symptoms, falls, infections, and the overall impact of the disease on the patient’s condition and the quality of life for caregivers and family.


  • Develop a personalized care plan in consultation with the patient, family, and other members of the Advanced Illness team, including the Primary Care Physician.
  • Ensure patients receive necessary care when needs are identified in accordance with the care plan.
  • Participate in evening/weekend on-call duties as required, providing on-call services competently and responsively.
  • Prescribe medications, including controlled substances, as permitted by state law. Order and interpret laboratory, imaging, and other diagnostic tests within the scope of practice. Order treatments and durable medical equipment as necessary.
  • Communicate clinical findings to the PSC Physician as needed, but at least bi-weekly. Regularly reassess the effectiveness of the care plan and modify as necessary.
  • Maintain effective working relationships with the supportive care team (e.g., Social Worker and RN Care Manager) and involve team members when the patient’s care plan requires collaborative participation to achieve desired outcomes.
  • Establish and maintain effective working relationships with all contacts during the course of work.
  • Document clinical findings, treatment plans, and care provided in the patient’s medical record in a manner consistent with accepted standards to support sound medical practice and reimbursement for services rendered.
  • Be knowledgeable about Palliative Medicine Quality Measures collection, documentation, and reporting.
About You

Qualifications for the Advanced Practice Nurse:

Essential Skills:

  • Strong leadership, interpersonal, written, and verbal communication skills.
  • Bilingual candidates preferred (Spanish).
  • Ability to make independent decisions and self-motivate.
  • Understanding of principles of Palliative Medicine, including criteria for home health or hospice care; familiarity with Medicare/Medicaid regulations; and interdisciplinary processes; knowledge of value-based and/or accountable organization requirements.
  • Compliance with mandatory continuing education requirements.
  • Proficiency with electronic medical record software.

Education and Experience:

  • Minimum of a Master of Science in Nursing from an accredited institution, or an equivalent as permitted by state law.
  • At least two years of experience as a Nurse Practitioner.
  • Preferred specialty in Family or Adult Medicine.
  • Experience in Value-Based Care practices is a plus.
  • Familiarity with Part B billing practices.
  • Ability to work autonomously in an Advanced Practice Role.

Licenses and Certifications:

  • Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
  • Current Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician as required by state law.
  • Part B Provider certification.
  • Federal DEA license.
  • State DEA license (if applicable).
  • ACHPN - Certification in Hospice and Palliative Care preferred.
  • Valid driver's license and current automobile insurance.
