The Nurse Practitioner in Advanced Illness Management (AIM) plays a crucial role in overseeing advanced illness management and palliative care for Curo patients, ensuring personalized treatment plans while exhibiting exceptional clinical expertise and ethical judgment.

This role involves providing diagnosis, treatment, consultation, and follow-up care under the guidance of the Palliative Medical Director or Primary Care physician.


We are seeking a Per Diem Nurse Practitioner to cover the Western Portion of the DFW Territory (Tarrant County area).


Key Responsibilities:

  • Collaborate with the Professional Services Corporation (PSC) Physician or supervising physician, Primary Care Physician, and other palliative care team members to deliver comprehensive advanced illness management and palliative care.
  • Assume responsibility for the entire process of advanced illness management from initial assessment to discharge.

Assessment should include:

evaluation of disease-specific changes, functional decline, nutritional status, cognitive function, pain management, and the overall impact of illness on the patient's and family's quality of life.


  • Develop an individualized care plan in consultation with the patient, family, and Advanced Illness team members, including the Primary Care Physician.
  • Ensure that necessary care is provided when identified in accordance with the care plan.
  • Participate in evening and weekend on-call duties as required, providing responsive and competent care.
  • Prescribe medications, including controlled substances, as permitted by state law, and order diagnostic tests as needed.
  • Regularly communicate clinical findings to the PSC Physician and reassess the effectiveness of the care plan.
  • Maintain effective working relationships with the supportive care team and involve them as necessary to achieve optimal patient outcomes.
  • Document clinical findings, treatment plans, and care provided in the patient's medical record in accordance with established standards.
  • Familiarity with Palliative Medicine Quality Measures and their documentation is essential.
About You

Nurse Practitioner Qualifications:

Essential Skills:

  • Strong leadership, interpersonal, and communication skills.
  • Ability to make independent decisions and demonstrate self-motivation.
  • Understanding of Palliative Medicine principles, Medicare/Medicaid regulations, and interdisciplinary processes.
  • Compliance with continuing education requirements.
  • Proficiency in electronic medical record systems.

Education and Experience:

  • Master of Science in Nursing from an accredited institution or equivalent.
  • At least two years of experience as a Nurse Practitioner.
  • Preferred specialty in Family or Adult Medicine.
  • Experience in Value-Based Care practices is a plus.
  • Familiarity with Part B billing practices.
  • Ability to work independently in an Advanced Practice Role.

Licenses and Certifications:

  • Current registered nurse license or equivalent as required by state law.
  • Valid Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician as required by state law.
  • Part B Provider certification.
  • Federal and state DEA licenses as required.
  • ACHPN certification in Hospice and Palliative Care is preferred.
  • Valid driver's license and automobile insurance.
