Job Summary:

Summit Health, Inc is seeking a highly skilled Advanced Practice Provider to join our team in Plano, TX. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the success of our organization.

Responsibilities:

  • Provide comprehensive and compassionate patient care, including conducting health histories, physical assessments, and developing treatment plans.
  • Work autonomously and collaboratively with other healthcare professionals to ensure seamless patient care.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to promote positive health outcomes.
  • Stay up-to-date with the latest medical research, technologies, and best practices to provide evidence-based care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Graduate of an accredited advanced practice program.
  • Current, unrestricted license to practice in the state of Texas or eligibility for licensure.
  • Active DEA registration or eligibility for registration.
  • Active Medicare/Medicaid enrollment or eligibility to enroll.
  • Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
  • Strong communication and interpersonal skills.
  • Experience with Electronic Medical Record (EMR) systems, preferably Athena.
  • Familiarity with Microsoft Office.
