Job Overview

Join the dedicated team at WeCare Centers as a Patient Admissions Specialist. In this essential role, you will contribute significantly to the operational efficiency of our facility by delivering outstanding service to residents, their families, and our staff.

Key Responsibilities
  • Welcome residents warmly upon their arrival.
  • Ensure rooms are prepared for new admissions and readmissions, which includes:
    • Collaborating with housekeeping and maintenance to resolve any room-related concerns.
    • Conducting thorough inspections to confirm cleanliness and readiness of equipment.
    • Providing welcome bags for each new resident.
    • Displaying welcome signs on doors for new admissions.
  • Facilitate the completion of Admission Agreements with residents or their representatives.
  • Communicate vital information and updates across departments to maintain seamless operations.
  • Send email notifications regarding new admissions and readmissions to relevant teams, detailing essential medical information.
  • Maintain an accurate and current roster of room assignments.
  • Input new admissions into the facility's electronic medical records (EMR) system.
  • Collect and upload necessary documentation into the EMR.
  • Conduct coverage eligibility verification and ensure documentation is uploaded appropriately.
  • Assign and upload Health Care Proxy/Power of Attorney documentation to the EMR, if available.
  • Update contact information following the receipt of signed Admission Agreements.
  • Perform additional duties as assigned.
