Overview of the Patient Transition Specialist Role

Become a vital member of our organization as a Patient Transition Specialist, delivering crucial support during a significant phase in a residential environment. In this temporary role, you will be instrumental in guiding patients from medical facilities back to their homes. This position is not for everyone; you will represent our organization and serve as the initial contact for families entering the realm of home healthcare. Your contributions are invaluable, making this a unique opportunity.

Core Duties of Patient Transition Specialists
  • Experienced nurses assigned by geographical area to assist in the transition of patients from medical facilities to home settings
  • Engagement on primary referrals exclusively, including partnerships with pediatric hospitals and health plans
  • Anticipation of complete transition from the home after a designated service period, with an additional grace period
  • Designated to a specific local area, with the potential to assist adjacent regions as required
Qualifications for Patient Transition Specialists
  • At least one year of nursing experience is required
  • Approval for the Transition Team by an internal registered nurse during the interview process
  • Validation of competencies in essential nursing skills, including Trach care, Ventilator management, G-Tube care, and Accurate Documentation
  • Exceptional customer service, communication, and professionalism are essential
  • Understanding of privacy laws such as HIPAA and patient confidentiality is crucial
  • Ability to adapt to fluctuating schedules, travel times, and diverse home environments
  • Willingness to work a minimum of four weekend shifts each month
Compensation and Benefits for Patient Transition Specialists
  • Commencing with five days of paid time off, with further accrual based on hours worked
  • Incentives available for peer referrals for nursing roles
  • Novice nurses are not eligible for the Transition Team
