Position Overview:

The Clinical Nurse Leader (CNL) operates as an advanced generalist, delivering and overseeing care for patients, families, and communities within a defined microsystem.

Key Responsibilities:

Evidence-Based Practice:
  • Facilitates comprehensive, evidence-based education for patients and families, addressing social and psychosocial barriers while promoting health equity.
  • Assesses and implements evidence-based practices, utilizing quality improvement strategies to enhance outcomes.
  • Collaborates in the development and evaluation of care practices relevant to the patient population.
  • Provides education and competency validation, mentoring both new and experienced staff through departmental initiatives.

Relationships and Caring:
  • Acts as an advocate and resource for patient care coordination within the microsystem.
  • Collaborates to establish complex patient and family goals, enhancing overall patient outcomes.
  • Reviews and customizes patient care plans with the interdisciplinary team to minimize complications.
  • Identifies and addresses variances in patient care processes, developing tailored plans to overcome barriers.

Critical Thinking:
  • Recognizes safety factors for patients and staff, implementing strategies to ensure a safe care environment.
  • Guides staff and students in applying critical thinking to manage complex patient scenarios.

Technical Expertise:
  • Collaborates in designing and implementing processes that leverage new technologies to improve patient care.
  • Evaluates patient cases for compliance with nursing standards, reporting findings to leadership.
  • Develops educational programs to enhance clinical expertise among staff.

Leadership:
  • Maintains communication with leadership to identify trends affecting quality care delivery.
  • Participates in initiatives for maintaining accreditation and specialty designations.
  • Supports academic programs to enrich educational experiences.
  • Engages in care coordination activities to facilitate patient transitions through the healthcare continuum.
  • Serves as a role model, recognizing and encouraging peer advancement.

Qualifications:
  • Current Professional Registered Nurse licensure in the State of Illinois.
  • Graduate degree in nursing.
  • National certification as Clinical Nurse Leader required upon hire or enrollment in an accredited nursing program.
  • Minimum of two years of relevant nursing experience.
  • BLS and other relevant emergency care certifications.
  • Proven track record in leading quality initiatives within a microsystem.
  • Strong communication skills and ability to manage time effectively.
  • Proficient in computer skills, including Excel and PowerPoint.
  • Ability to navigate electronic medical records.
  • Demonstrated knowledge of population-specific clinical practices.
