Role Summary:
The Social Services Specialist is responsible for delivering psychosocial support to residents and their families.

Qualifications:

  • Education: Bachelor’s degree in Social Work (BSSW) or Master’s degree in Social Work (MSSW) preferred.
  • Licenses/Certification: Must hold a valid license as required by state regulations.
  • Experience: Minimum of one year in a long-term care setting.

Key Responsibilities:

  • Deliver direct psychosocial interventions.
  • Conduct resident assessments during admission, upon changes in condition, and annually.
  • Develop, review, and update care plans and progress documentation.
  • Coordinate resident appointments with external services.
  • Participate in and document resident council meetings.
  • Support families in navigating skilled nursing placements and coping with health challenges.
  • Collaborate with residents, families, and team members for discharge planning.
  • Facilitate in-service training for staff on psychosocial topics and patient rights.
  • Supervise and mentor Social Services Assistants.
  • Perform additional duties as assigned.

  • Effective communication skills with residents, families, and all organizational levels.
  • Commitment to maintaining confidentiality.
  • Knowledge of psychosocial practices relevant to long-term care.
  • Proficient in guiding and motivating team members.
