Position Summary:
The Licensed Social Worker provides essential psychosocial support to residents and their families.

Qualifications:

  • Education: Bachelor’s or Master’s degree in Social Work preferred.
  • Licenses/Certification: Must hold a valid license as required by state regulations.
  • Experience: Minimum of one year in a long-term care setting.

Key Responsibilities:

  • Deliver direct psychosocial interventions.
  • Conduct resident assessments during admission, upon changes in condition, and annually.
  • Develop, review, and update care plans and progress notes.
  • Coordinate resident visits with external services, including dental and optical care.
  • Participate in and document resident council meetings.
  • Support families in navigating skilled nursing placements and coping with illness and loss.
  • Collaborate with patients, families, and team members to facilitate discharge planning.
  • Provide in-service training to staff on psychosocial topics and patient rights.
  • Supervise and mentor Social Services Assistants.
  • Complete additional tasks as assigned.
