Position Summary:
The Licensed Social Worker provides essential psychosocial support to residents and their families.
Qualifications:
- Education: Bachelor’s degree in Social Work (BSSW) or Master’s degree in Social Work (MSSW) preferred.
- Licenses/Certification: Must hold a valid license as required by the state.
- Experience: Minimum of one year in a long-term care setting.
Key Responsibilities:
- Deliver direct psychosocial interventions.
- Conduct resident assessments during admission, upon changes in condition, and annually.
- Develop, review, and update care plans and progress notes.
- Coordinate resident visits with external services, including dental and optical care.
- Document and participate in resident council meetings.
- Assist families in navigating the challenges of skilled nursing placement and related issues.
- Collaborate with patients, families, and team members to facilitate discharge planning.
- Provide in-service training to staff on psychosocial topics and patient rights.
- Supervise and mentor Social Services Assistants.
- Complete additional tasks as assigned.
Knowledge, Skills, and Abilities:
- Ability to work effectively as part of a team.
- Strong communication skills with residents, families, and all organizational levels.
- Commitment to maintaining confidentiality.
- Understanding of psychosocial practices relevant to long-term care.
- Proficient in guiding and motivating team members.
