Position Summary:
The Licensed Social Worker provides essential psychosocial support to residents and their families.

Qualifications:

  • Education: Bachelor’s degree in Social Work (BSSW) or Master’s degree in Social Work (MSSW) preferred.
  • Licenses/Certification: Must hold a valid license as required by the state.
  • Experience: Minimum of one year in a long-term care setting.

Key Responsibilities:

  • Deliver direct psychosocial interventions.
  • Conduct resident assessments during admission, upon changes in condition, and annually.
  • Develop, review, and update care plans and progress notes.
  • Coordinate resident visits with external services, including dental and optical care.
  • Document and participate in resident council meetings.
  • Assist families in navigating the challenges of skilled nursing placement and related issues.
  • Collaborate with patients, families, and team members to facilitate discharge planning.
  • Provide in-service training to staff on psychosocial topics and patient rights.
  • Supervise and mentor Social Services Assistants.
  • Complete additional tasks as assigned.

  • Strong communication skills with residents, families, and all organizational levels.
  • Commitment to maintaining confidentiality.
  • Understanding of psychosocial practices relevant to long-term care.
  • Proficient in guiding and motivating team members.
