Our case management services are provided through the Community First Choice (CFC) and the Developmental Disability Administration (DDA) programs, where our case managers play a vital role in assisting participants with the coordination and continuity of care.

Required Qualifications:
- A bachelor's degree in Human Services, such as sociology, nursing, counseling, or a related field is preferred, OR
- A bachelor's degree in another field may be considered with relevant work experience,
- Experience in case management for individuals with complex medical and/or behavioral health needs, older adults, and/or children and youth with disabilities is essential, with a minimum of:
- 6 months for a BA in Human Services, or
- 1 year for a BA in other fields, OR
- An associate degree with at least 2 years of experience in the human services field, OR
- A high school diploma with a minimum of 7 years of experience in the human services field.
- A minimum typing speed of 30 WPM.
- Proficient in computer skills.
- A valid Maryland driver's license and an insured vehicle are required.
Responsibilities of Case Managers:
- Utilizing training knowledge to evaluate a participant's medical, physical, and environmental conditions, assisting them in coordinating services tailored to their individual needs.
- Engaging with participants, family members, and service providers to ensure seamless service delivery as outlined in the care plan.
- Regularly assessing and monitoring services to guarantee the highest quality of care.
- Meeting documentation requirements effectively.
- Adhering to State, Federal, and Contract guidelines to ensure compliance with all program regulations.
