Job Summary

Community Options is seeking a highly skilled Intake Coordinator to join our team. As the point person for program admissions, you will be responsible for coordinating and facilitating the admission process for new individuals and ensuring existing individuals remain current and in good standing.

Key Responsibilities
  • Collect and upload necessary admission documentation, including SSA award letters, birth certificates, Medicaid cards, and IDs.
  • Ensure service authorizations in Therap are created and maintained to avoid loss of services and payments.
  • Attend pre-placement meetings for new admissions to ensure all necessary documents are collected prior to admission.
  • Enroll and admit individuals into Therap.
  • Update and enter new residents and amendments to services in Therap.
  • Assist directors with completing rep payee, room and board, eligibility determination, and other pertinent applications/contracts.
  • Enter, review, and update authorizations for all individuals.
  • Generate weekly attendance and ISP data in Therap and communicate completion to the billing department.
  • Collaborate with the billing department and regional offices to troubleshoot issues impacting billing and cash flow.
Requirements
  • Excellent verbal, written, interpersonal, and communication skills.
  • Knowledge and understanding of local and regional regulatory operations.
  • Knowledge and understanding of state regulations.
  • Understanding and commitment to community-based support for persons with disabilities.
  • Experience with intake and admissions.
  • Excellent time management skills.
  • Familiarity with Social Security and Medicaid.
  • Working knowledge of ICD-10 codes.
  • Therap experience is a plus.
  • Proficiency in Microsoft Outlook, Word, and Excel.
  • High school diploma or equivalent.
  • Ability to establish a comfortable and supportive relationship with individuals receiving support and team members.
  • General understanding of ODP and Waiver services preferred.
  • Experience with HCSIS and Promise is a plus.
Working Conditions
  • May be required to be on-call in cooperation with other management staff.
  • Moderate overtime required throughout the year.
  • Minimal hazards, general office working conditions.
  • Valid driver's license with a good driving record.
