Overview: Under the guidance of the HIV Prevention Program Provider/Director and in collaboration with the Medical Services Director, the role involves community outreach, education, and case management services aimed at at-risk African American and Latinx populations residing in South Los Angeles. A strong understanding of the local community and experience working with at-risk groups is essential for networking, educating, raising awareness, recruiting, and enrolling eligible individuals into HIV prevention programs. Participation in provider and community meetings is crucial for increasing awareness and establishing referral networks with other organizations. Responsibilities include conducting rapid HIV testing and counseling, facilitating group sessions, providing individual support, and making referrals to HIV care as necessary.
  • Conducts personalized and confidential assessments for clients regarding risk and medication adherence to support reduction and adherence efforts.
  • Develops resources for PrEP/PEP services for clients in need and establishes connections with PrEP/PEP providers.
  • Ensures access to HIV prevention methods, including biomedical prevention services (PrEP/PEP), while providing follow-up and support to enhance risk reduction and behavior change.
  • Facilitates access to screening for STI/STD's and ensures appropriate referrals.
  • Collaborates with project staff to promote SCFHC's HIV prevention and treatment initiatives.
  • Conducts rapid HIV testing and counseling at various community venues, including mobile units and health fairs.
  • Follows up on referrals to community services effectively.
  • Refers individuals diagnosed with HIV to case management, medical care, and supportive services, ensuring thorough follow-up and documentation of all referrals.
  • Provides accurate information regarding HIV transmission, prevention, and testing to the community.
Qualifications:
  • Bachelor's Degree in a health-related field or equivalent years of relevant educational/work experience.
  • Preferred experience in substance abuse, human services, or social service environments.
  • State certification in HIV testing and counseling is desirable.
  • Completion of the American Academy of HIV Medicine non-provider curriculum within six months of hire is required.
  • Completion of National STI/STD & HIV Curriculum within six months of hire is required.
  • Two to four years of experience in providing human services, health care services, or outreach services is preferred.
  • Experience with data entry and collection, and presenting information in both graphic and oral formats.
  • Formal training in HIV education and prevention is highly valued.
  • Valid California Class "C" driver's license is required.
  • Proficiency in Microsoft Office products (Word/Excel), communication systems, and internet search capabilities.
  • Bilingual and bi-literate in Spanish/English is preferred.
