Role Summary

The Care Manager plays a pivotal role in guiding individuals through the complexities of Long-Term Services and Supports. This position is essential in orchestrating the collaboration between formal and informal caregivers to ensure clients receive comprehensive care. Our approach to Care Management prioritizes the preferences and rights of the consumer, aligning with the mission of our organization.

Key Responsibilities

  1. Conduct thorough, individualized assessments in a person-centered manner, involving family or representatives as needed, in the client’s preferred environment.
  2. Provide education on program components, service options, and guidelines, including eligibility criteria, costs, and how these interact with the client's support systems.
  3. Ensure timely completion of all necessary client documentation, applications, and related paperwork.
  4. Continuously evaluate the effectiveness, quality, and appropriateness of care plans and service delivery, making adjustments in collaboration with service providers as required.
  5. Engage in person-centered interviews with clients and their families to reassess needs and adapt care plans accordingly.
  6. Collaborate effectively within an interdisciplinary team and with external resources, participating in on-call services as needed.
  7. May assist in training new staff and contribute to ongoing educational initiatives.

Qualifications

Education:

A Bachelor's degree in administration, social work, public health, psychology, counseling, gerontology, or a related field is required.

Experience:

  • A minimum of two years in healthcare or human services is required. A Bachelor's degree in Nursing, Health, Social Work, Gerontology, or a related field may substitute for one year of experience.
  • Understanding of psychological, developmental, social, health, and economic factors that influence individual and family behaviors.
  • Familiarity with community resources and the ability to integrate them into a cohesive care plan.
  • Knowledge of funding sources, including Medicaid and Medicare.
  • Experience in conducting comprehensive, person-centered assessments in various settings.
  • Strong advocacy and communication skills.
  • Commitment to the principles of person-centered care and independent living.
  • Proven ability to build rapport with clients, families, and community resources.
  • Experience in interviewing, observation, and analytical skills.

Other Requirements:

Must possess reliable transportation, a valid driver's license, and current automobile insurance.

Work Environment and Physical Requirements
