Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at Optum, where you will play a critical role in delivering high-quality, patient-centered care to individuals with complex chronic conditions in the comfort of their own homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Provide ongoing care and support to patients, including medication management, wound care, and education on disease management and wellness.
  • Collaborate with a multidisciplinary team of healthcare professionals to ensure seamless care coordination and communication.
  • Prescribe diagnostic tests and interventions as needed to prevent unnecessary hospitalizations and improve patient outcomes.
  • Stay up-to-date with the latest medical research, technologies, and best practices in home-based medical care.
Requirements
  • Master's degree in Nursing or related field.
  • Current state RN/NP license and certification through a recognized professional organization.
  • At least 2 years of experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Knowledge of long-term care, community resources, and cost-effective alternatives in patient care delivery systems.
  • Ability to work independently and as part of a team to achieve patient-centered care goals.
