- Master's degree in Nursing or related field.
- Current state RN/NP license and certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- At least 2 years of experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives in patient care delivery systems.
- Ability to work independently and as part of a team to achieve patient-centered care goals.
