  • Clinical Updates: Provide timely clinical updates to third-party case managers to facilitate ongoing insurance authorization.
  • Documentation Collaboration: Work closely with the interdisciplinary team to gather necessary documentation for admissions.
  • Liaison Role: Act as a liaison between discharging facilities and our organization to support census stability and growth.
  • Equipment Preparation: Ensure that all necessary equipment is obtained and that care staff are trained in new care techniques prior to patient admission.
  • Room Selection Review: Evaluate room selections based on medical history and compatibility for roommate assignments.

Qualifications:
  • Graduate from an accredited nursing program (LPN or RN).
  • Current unrestricted RN/LPN license in the relevant state.
  • Three to five years of clinical experience, including supervisory experience in a long-term care setting preferred.
  • Strong observation, communication, administrative, leadership, and supervisory skills.
  • Ability to meet physical demands of the role.
