Position Summary:
The Service Coordinator plays a pivotal role in orchestrating programs and services that empower clients or residents to achieve optimal independence and well-being, both physically and mentally.

Key Responsibilities:

  • Guide and inform clients or residents and their families in accessing community resources essential for a self-sufficient lifestyle.
  • Serve as a bridge between community organizations, service providers, and clients or residents, actively seeking new services and negotiating affordable options.
  • Oversee the provision of services to clients or residents, ensuring they are delivered timely and meet quality standards.
  • Maintain an updated directory of community resources available to clients or residents.
  • Foster professional relationships with clients or residents to identify needs and facilitate referrals to appropriate agencies.
  • Keep comprehensive resident files, including intake assessments, service referrals, and follow-up documentation.
  • Conduct quarterly reviews of service quality and implement follow-up actions as necessary.
  • Ensure timely submission of all required reports and data.
  • Network with service providers to strengthen relationships and explore additional service opportunities.
  • Travel may be required for conferences, training sessions, and other organizational events.
  • Perform additional duties as assigned.
Specific Duties for UCH Engage Housing Service Coordinator:
  • Encourage wellness initiatives for residents, including organizing health screenings and promoting proactive health management.
  • Coordinate educational workshops for residents, families, and staff regarding available community resources and eligibility requirements.
  • Assist residents in forming informal support networks with peers and family.
  • Facilitate volunteer programs in collaboration with local service organizations.
  • Execute all essential functions outlined in the general responsibilities.
Specific Duties for NaviGuide Community Service Coordinator:
  • Evaluate client needs to develop personalized service plans.
  • Act as a support system for families, providing necessary community resources for caregivers.
  • Conduct home visits as needed, even in adverse weather conditions.
  • Document all meetings and communications with clients, families, and providers.
  • Daily travel to client homes and community service providers is required.
  • Execute all essential functions outlined in the general responsibilities.

Qualifications:
  • Education: Bachelor’s Degree in Social Work preferred.
  • Experience: Minimum of two years in social service delivery, particularly with the elderly population.
  • Technical Skills: Proficient in Microsoft Office and other relevant software.
  • Certifications: Familiarity with the American Association of Service Coordinators is advantageous.
  • Other Requirements: Strong interpersonal skills, ability to work independently, and adherence to organizational policies.
