Position Summary: The Case Manager plays a vital role in assisting the Clinical Coordinator with comprehensive client services. This includes developing treatment plans, managing case loads, facilitating both group and individual interventions, addressing crises, coordinating care, and planning for client discharge.

Key Responsibilities:

  • Create and implement personalized treatment plans; track, assess, and document client progress.
  • Manage a designated client caseload, ensuring timely completion of all necessary documentation, including progress notes and treatment plans, in compliance with regulatory standards and company policies.
  • Provide individual and group interventions, including crisis management as needed.
  • Coordinate care through case reviews and weekly updates on treatment progress, as well as aftercare services.
  • Engage in clinical meetings, training sessions, and other required activities.
  • Address program requirements and conflicts with clients effectively.
  • Assist clients in navigating applications for government assistance and referrals to external agencies.
  • Uphold internal policies and adhere to behavioral health licensure, as well as local, state, and federal regulations.
  • Perform additional duties as assigned.
Qualifications:
  • Must fulfill Arizona Department of Health Services personnel requirements, including being at least 21 years old or possessing relevant licensure or certification.
  • Demonstrate high ethical standards, sound judgment, problem-solving abilities, and strong organizational skills.
  • Possess a valid Fingerprint Clearance Card.
  • Proficient in Microsoft Office, EMR systems, and typing.
  • Successfully pass a background check.
  • Hold current CPR/First Aid/Narcan Certification.
  • Maintain an annual negative TB test.
Knowledge, Skills, and Abilities:
  • Familiarity with addiction recovery processes and twelve-step programs.
  • Strong written and verbal communication skills, with the ability to engage effectively with diverse audiences.
Education & Experience:
  • High School diploma or GED is required.
  • 1-2 years of relevant experience or training, or an equivalent combination of education and experience.
Work Environment:
  • Fast-paced behavioral health setting.
  • Primarily indoors, with occasional outdoor responsibilities.
Physical Demands:
  • Regularly required to sit, speak, and hear; occasionally required to stand, walk, and lift up to 50 pounds.
  • Specific vision abilities required include color vision and depth perception.
Shift Requirements:
  • Availability for days, nights, weekends, and holidays.
Travel:
  • Valid AZ Driver's License required; may involve travel between sites.
