Company Overview: Crossroads Inc. is a licensed provider of substance abuse treatment services, dedicated to assisting individuals, including Veterans, who are struggling with Substance Use Disorders. We pride ourselves on delivering effective and affordable recovery programs, both residential and outpatient.
Position Summary: The Case Manager plays a crucial role in supporting the Clinical Coordinator by managing client services, which encompasses treatment planning, case load oversight, individual and group interventions, crisis management, care coordination, and discharge planning.
Key Responsibilities:
- Develop and implement personalized treatment plans; track, assess, and document client progress.
- Maintain a designated client caseload, ensuring timely completion of all necessary documentation, including progress notes and treatment plans, in line with regulatory standards and company policies.
- Provide individual and group interventions, including crisis management as needed.
- Facilitate care coordination through case reviews and regular updates on treatment progress and aftercare services.
- Participate in clinical meetings, training sessions, and other required activities.
- Address program requirements and client conflicts effectively.
- Assist clients with applications for government assistance and referrals to external agencies.
- Comply with internal policies and all relevant behavioral health regulations.
- Perform additional duties as necessary.
- Must fulfill Arizona Department of Health Services personnel requirements, including being at least 21 years old or holding relevant licensure.
- Uphold high ethical standards and demonstrate strong judgment, problem-solving abilities, and organizational skills.
- Possess a valid Fingerprint Clearance Card.
- Proficient in Microsoft Office, EMR systems, and typing.
- Successfully pass a background check.
- Hold current CPR/First Aid/Narcan certification.
- Maintain an annual negative TB test.
- Familiarity with addiction recovery and twelve-step programs.
- Strong written and verbal communication skills, with the ability to engage effectively with diverse audiences.
- High School diploma or GED is required.
- 1-2 years of relevant experience or training, or an equivalent combination of education and experience.
- Fast-paced behavioral health setting.
- Primarily indoors, with occasional outdoor tasks.
- Regularly required to sit, speak, and hear; occasionally required to stand, walk, and lift/move up to 50 pounds.
- Availability for days, nights, weekends, and holidays.
- Valid AZ Driver's License required; may involve travel between various sites.
