Experienced HIV Care Coordinator

The Institute for Family Health is seeking a skilled Compassionate Nurse Care Manager to join our team. As a key member of our interdisciplinary team, you will play a vital role in coordinating care for HIV positive patients, addressing barriers to treatment adherence, and promoting viral load suppression and retention in care.
Key Responsibilities- Meet with HIV positive patients to assess their needs and develop individualized care plans
- Perform clinical nursing duties, including triaging patients, administering medications, and performing bloodwork
- Screen patients for mental health and substance use treatment needs, and refer them to appropriate services
- Collaborate with medical providers, nursing staff, and other Institute employees to coordinate care and ensure seamless transitions
- Provide individual and group-based health promotion, treatment adherence counseling, and risk reduction counseling to patients
- Regularly review health maintenance topics and outreach patients to ensure compliance with care plans
- Complete prior authorizations to ensure timely access to necessary medications
- Coordinate with pharmacies on behalf of patients
- Participate in initial assessments of service needs and develop comprehensive individualized service plans
- Valid NYS nursing license; RN required
- Computer literacy with proficiency in MS Word, Excel, and PowerPoint
- Demonstrated organizational, interpersonal, oral, and written communication skills
- Ability to generate, interpret, and analyze data from multiple sources
- Bi-lingual, Spanish preferred
