HIV Care Coordinator

Role Summary:
We are seeking a dedicated and professional full-time Patient Navigator to join our team. The ideal candidate will possess strong interpersonal skills, effectively engage with a diverse clientele, and communicate proficiently with healthcare providers and support personnel. The Patient Navigator will serve as the primary liaison for patients enrolled in our HIV care coordination program, fostering meaningful relationships to ensure they remain connected to essential prevention and treatment services.
Key Responsibilities:
- Deliver direct support services to individuals living with HIV/AIDS under the guidance of the Care Coordinator.
- Execute tasks necessary for implementing medical and support service plans, including accompanying patients to appointments, providing patient coaching, and conducting health education sessions.
- Conduct field-based tasks such as monitoring patient adherence to antiretroviral therapy (ARVs), assessing and reporting any side effects, and providing support in various convenient locations.
- Maintain accurate client records and documentation in accordance with the standards set by Apicha's Client Services Department.
- Collaborate with the care team, which may include physicians, nurses, and other healthcare professionals, to optimize patient health outcomes.
- Represent the organization at community events to promote our services.
- Perform additional duties as assigned.
Qualifications:
- Associate's degree in human services or social work, with a minimum of 12 months of experience in patient navigation or a related field.
- Consideration may be given to candidates with peer worker certification from the NYS AIDS Institute or case management certification from CUCS, or equivalent training.
- Strong cultural competency and experience working with the target population are essential.
- Fluency in one or more Asian languages is preferred.
- Excellent written and verbal communication skills in English are required.
- Proof of immunization or immunity to specific communicable diseases and tuberculosis testing is mandatory, as per health regulations.
