Key Responsibilities:
- Client Assessment: Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
- Care Coordination: Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination and effective communication.
- Education and Support: Educate clients and their families on their conditions, treatment options, and self-care techniques to promote independence and well-being.
- Documentation and Reporting: Maintain accurate and detailed records of client interactions, care plans, and progress to ensure compliance with regulatory requirements.
- Team Collaboration: Work closely with our interdisciplinary team to ensure comprehensive care and support for our clients.
Requirements:
- Experience: At least 3 years of experience in patient care coordination or a related field.
- Education: High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
- Language Skills: Fluent Spanish language skills required.
- Communication Skills: Excellent verbal and written communication skills to effectively interact with clients, families, and healthcare providers.
- Computer Skills: Proficient in Microsoft Office and other relevant software applications.
