Established in 1965, the Chinese-American Planning Council, Inc. (CPC) is dedicated to fostering the social and economic empowerment of Chinese American, immigrant, and low-income populations. As the largest Asian American social services agency in the nation, CPC operates over 50 programs across more than 35 locations in New York City. In 2022, CPC's core programs and emergency services reached approximately 200,000 individuals across all five boroughs.

PROGRAM OVERVIEW

The Community Health Services (CHS) program is focused on raising awareness about HIV/AIDS within the Asian American and Pacific Islander (AAPI) communities. Through educational outreach, prevention information, and advocacy, CHS provides essential support and referrals for HIV testing. Utilizing cultural and linguistic expertise, the program ensures quality care for those affected by HIV/AIDS, working to eliminate stigma and promote health equity.

POSITION SUMMARY

The part-time Case Manager will report directly to the Program Director and is responsible for delivering essential support services to individuals living with HIV/AIDS. This role includes linking clients to necessary care, recruiting high-risk individuals for testing, and engaging in evidence-based interventions.

KEY RESPONSIBILITIES

  • Facilitate comprehensive service coordination for affected individuals and high-risk populations, ensuring they receive the necessary information and support to navigate the healthcare system and community resources.
  • Conduct field visits and provide translation services as needed.
  • Perform ongoing assessments of community members' situations through documentation and progress notes.
  • Assist in accessing and coordinating social services benefits for community members.
  • Prepare and submit reports in compliance with government funding requirements.
  • Support recruitment efforts targeting at-risk demographics for local interventions.
  • Establish and maintain collaborative relationships with medical and social service providers.
  • Design and deliver workshops to educate the community on HIV/STD issues.
  • Represent the program at external events and collaborate with other organizations and funding sources.
  • Participate in special projects as assigned by the Program Director.

QUALIFICATIONS

Education and Experience:

  • A Bachelor's Degree in Social Work or a related field is preferred.
  • Experience working with HIV-affected populations and/or within Chinese/Asian communities is advantageous, along with a strong interest in community education.

Skills and Competencies:

  • Bilingual proficiency in Mandarin and English is essential.
  • Strong written and verbal communication skills, with the ability to work effectively in a team environment.
  • Flexibility in scheduling is preferred.

COMPENSATION & BENEFITS

  • Hourly wage of $22.53 for 21 hours per week.
