Position Summary:

The Nurse Care Coordinator plays a vital role within a multidisciplinary team, focusing on the management of care for patients living with HIV. This position is essential in overcoming obstacles to treatment adherence, achieving viral load suppression, and ensuring continuity of care. The Nurse Care Coordinator is responsible for executing clinical nursing tasks for patients during their medical appointments, which includes patient triage, vital sign assessment, medication administration, and conducting laboratory tests.

Key Responsibilities:
  • Engage with patients during their medical visits to provide comprehensive care.
  • Execute relevant clinical nursing functions to support patient health.
  • Assess patients for mental health and substance use issues, providing referrals as necessary.
  • Collaborate closely with healthcare providers and staff to ensure coordinated patient care through case discussions and joint visits.
  • Deliver individual and group health promotion, treatment adherence support, risk reduction strategies, and case management to help patients overcome barriers to care.
  • Regularly review health maintenance topics and proactively reach out to patients for compliance.
  • Complete prior authorizations to facilitate timely access to medications.
  • Coordinate with pharmacies to assist patients with their medication needs.
  • Participate in assessing service requirements and barriers, aiding in the development of personalized service plans, and following up to ensure implementation.
Qualifications:
  • Current New York State nursing license; RN designation required.
  • Proficient in computer applications including MS Word, Excel, and PowerPoint.
  • Strong organizational, interpersonal, and communication skills, with the ability to manage multiple tasks effectively.
  • Ability to analyze and interpret data from various sources.
  • Fluency in Spanish is preferred.
