Position Overview:
The Patient Navigator will play a crucial role within the Signature Addiction Medicine team, focusing on the coordination of care for individuals facing Substance Use Disorders. This position involves working closely with both internal and external partners to eliminate obstacles to care and facilitate seamless transitions for patients and their families. The Patient Navigator will actively engage with patients to identify their needs and connect them with suitable medical and social resources.

Key Responsibilities:
  1. Guide patients in accessing essential services and resources, including addiction treatment, recovery support, healthcare, and social services such as job training and housing assistance.
  2. Support patients in navigating their care and addressing social determinants of health by linking them to appropriate specialists and community resources.
  3. Establish and maintain relationships with community organizations to enhance outreach efforts.
  4. Assist patients in completing necessary documentation and paperwork.
  5. Provide administrative support at clinical sites, including scheduling appointments and verifying patient information.
  6. Coordinate transportation solutions for patients attending appointments and services.
  7. Assist in resolving insurance-related issues and securing authorizations as required.
  8. Collaborate with external referral sources to ensure proper connections to the Signature OBAT Program.
  9. Facilitate follow-up communications with patients regarding their medical appointments.
  10. Work closely with patients to relay vital information to the medical team for effective care.
  11. Partner with healthcare providers to promote adherence to care plans.
  12. Help patients and families address daily challenges that may impact their care.
  13. Provide timely updates regarding patient conditions to the care team.
  14. Assist in data collection and analysis to monitor performance metrics.
  15. Document patient interactions in the electronic medical record promptly.
  16. Support patients in connecting with community-based resources and services.
  17. Engage with patients who have missed appointments to encourage re-engagement in care.
  18. Collaborate with the Signature Healthcare OBAT Program and other community recovery initiatives.
  19. Uphold patient confidentiality and adhere to professional standards of conduct.
  20. Perform additional duties as assigned to support the team's objectives.
Qualifications:
  • Understanding of addiction treatment and healthcare systems, along with knowledge of community resources.
  • Strong communication skills, with the ability to build rapport and demonstrate empathy towards individuals with addiction and behavioral health challenges.
  • Resourceful and proactive in problem-solving.
  • Ability to manage multiple tasks and work effectively under pressure.
  • Proficient in reading and interpreting patient documentation.
  • Excellent verbal and written communication skills; proficiency in additional languages is a plus.
Education and Experience:
  1. High School diploma or equivalent required; Associate's degree preferred.
  2. 1-2 years of relevant experience in a similar role, with a preference for backgrounds in addiction or behavioral health.
  3. Current Medical Assistant Certification is required.
  4. Proficient in Microsoft Office and familiar with electronic medical records systems.
