Job Summary:

The Clinical Social Worker - Addiction Specialist will be a key member of the Signature Healthcare team, working closely with primary care providers to assess, plan, implement, coordinate, and monitor options and services to meet patients' whole healthcare needs. This role requires a strong understanding of substance use disorders and their treatment, as well as experience working in a medical setting.

Key Responsibilities:

  • Collaborate with primary care providers and other members of the multidisciplinary team to establish measurable healthcare goals and prioritize patients' needs.
  • Identify and evaluate patients with mental health and substance use disorders to provide appropriate care.
  • Work with community behavioral health partners to facilitate patient access to care.
  • Deal with patient crises as necessary.
  • Monitor substance use care provided to patients in the community.
  • Track relevant screenings and registries via the electronic medical record.
  • Consult with primary care providers on the impact of substance use on chronic medical conditions and plan how to address these issues.
  • Meet with patients and/or family members for short-term therapy when necessary.
  • Use evidence-based practices in treatment, such as solution-focused short-term therapy, motivational interviewing, and strength-based approaches.
  • Refer to and collaborate with community resources to locate concrete services for patients.
  • Attend team, departmental, and practice meetings.
  • Pursue ongoing education to maintain competence in practice.
  • Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
  • Communicate effectively with internal and external customers, respecting differences in cultures, values, beliefs, and ages, and utilizing interpreters when needed.

Requirements:

  • Master of Social Work degree or Master's in Mental Health Counseling degree from an accredited graduate program.
  • 3-5 years of experience in a medical setting, with a focus on substance use disorders and their treatment.
  • Knowledge of crisis intervention and evidence-based treatment techniques.
  • Ability to work in a fast-paced, multidisciplinary environment.
  • Ability to work independently, proactively, and take the initiative in decision-making.
  • Excellent time management skills.
  • Knowledge of community-based resources.
