Job Summary
We are seeking a highly skilled Clinical Social Worker to join our Signature Addiction Medicine/OBAT team. As a key member of our multidisciplinary team, you will work closely with primary care providers to assess, plan, implement, and monitor options and services to meet patients' whole healthcare needs.
Key Responsibilities
- Assess patients' medical, psychosocial, behavioral, cultural, and spiritual needs, and develop individualized care plans.
- Collaborate with primary care providers and other team members to establish measurable healthcare goals and prioritize patients' needs.
- Identify and evaluate patients with mental health and substance use disorders, and provide appropriate care and referrals.
- Work with community behavioral health partners to facilitate patient access to care and services.
- Deal with patient crises and provide crisis intervention services as needed.
- Monitor substance use care provided to patients in the community and track relevant screenings and registries via the electronic medical record.
- Consult with primary care providers on the impact of substance use disorders on chronic medical conditions and develop plans to address these issues.
- Provide short-term therapy to patients and/or family members as needed.
- Use evidence-based practices in treatment, such as solution-focused short-term therapy, motivational interviewing, and strength-based approaches.
- Refer patients to community resources and collaborate with these resources to provide concrete services.
- Attend team, departmental, and practice meetings to stay informed and contribute to the development of care plans.
- Pursue ongoing education to maintain competence in practice and stay up-to-date on best practices in addiction medicine and behavioral health.
Requirements
- Master of Social Work degree or Master's in Mental Health Counseling degree from an accredited graduate program.
- 3-5 years of experience in a medical setting, preferably in addiction medicine or behavioral health.
- Knowledge of crisis intervention and evidence-based treatment techniques.
- Ability to work in a fast-paced, multidisciplinary environment and take the initiative in decision-making.
- Excellent time management skills and ability to prioritize tasks effectively.
- Knowledge of community-based resources and ability to navigate the healthcare system.
