Position Summary:

The clinical social worker, as part of the SAM team, engages in a collaborative process with primary care providers which assesses, plans, implements, coordinates and monitors options and services to meet a patient’s whole healthcare needs. The social worker assists in the identification of appropriate substance use disorder(s) (SUD) and other community resources ensuring that available resources are being used in a timely and cost effective manner in order to obtain improved patient outcomes and an improved patient experience. The qualified candidate will have experience in a medical setting as well as expertise in evidence based practices relevant to working with patients and their families with co-occurring mental health and SUD. The social worker will demonstrate cultural competence in care delivery.

Department: Signature Addiction Medicine/OBAT

This is a full-time 40 hour/ week position

Responsibilities:

  • Addresses the total patient inclusive of medical, psychosocial, behavioral, cultural and spiritual needs.

  • Collaborates with the primary care providers and other members of the multidisciplinary team to establish measurable healthcare goals and prioritization of the patient’s needs as well as identifying any barriers to care.

  • Identifies and evaluates patients with mental health and SUD in order to provide appropriate care. Works with community behavioral health partners to facilitate patient access to care.

  • Deals with patient crises as necessary.

  • Monitors substance use care provided to SAM patients in the community.

  • Tracks relevant screenings and registries via the electronic medical record.

  • Consults with primary care providers around the impact of SUD on chronic medical conditions and plans how to address these issues.

  • Meets with patients and/or family members for short term therapy when necessary.

  • Uses evidence-based practices in treatment such as solution focused short term therapy, motivational interviewing and strength based approaches in treating patients.

  • Refers to and collaborates with the community resources to locate concrete services for patients.

  • Attends team, departmental and practice meetings.

  • Pursues ongoing education to maintain competence in practice.

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

  • Commits to recognize and respect cultural diversity for all customers (internal and external).

  • Communicates effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed.

  • Performs other duties as assigned.

BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

  • Comfortable with medical providers and medical terminology.

  • Excellent interpersonal, verbal and written communication skills.

  • Knowledge of crisis intervention and evidence based treatment techniques.

  • Knowledge of the interactions between mental illness/substance use disorders and physical illness.

  • Experience of substance use disorders and their treatment.

  • Ability to work in a fast paced, multidisciplinary environment.

  • Ability to work independently, proactively and take the initiative in decision making.

  • Ability to seek out ways to constantly improve care and identify gaps in service delivery.

  • Excellent time management skills.

  • Knowledge of community-based resources.

Education/Experience/Licenses/Technical/Other:

  • Education: Master of Social Work degree or Masters in Mental Health Counseling degree from an accredited graduate program

  • Experience (Type & Length): 3-5 years in a medical setting required

  • Certification/Licensure: LICSW or LMHC

  • Software/Hardware: Proficiency with Microsoft Office products, particularly MS Word and Excel
