Job Summary

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Signature HealthCARE. As a Patient Care Coordinator, you will play a vital role in ensuring the smooth transition of patients through our healthcare system, providing exceptional care and support to those in need.

Key Responsibilities
  • Assist patients in navigating the healthcare system, connecting them with appropriate medical and social resources.
  • Collaborate with internal and external entities to remove barriers to care and ensure seamless transitions for patients and families.
  • Work with patients to address and meet their needs, linking them to relevant medical and social services.
  • Provide administrative support, including scheduling appointments, obtaining and verifying contact information, and assisting with care plans.
  • Coordinate transportation to appointments and services as needed, and assist with resolving insurance issues and obtaining authorizations.
  • Work closely with the medical team to provide timely updates and changes in patient condition, and collaborate with providers to ensure adherence to care plans.
  • Document patient encounters in the electronic medical record in a timely manner, and assist with data collection and analysis for performance measures.
Requirements
  • High School graduate or equivalent required; Associates degree preferred.
  • 1-2 years of successful working experience in a similar capacity, with prior addiction or behavioral health clinical/program experience helpful and preferred.
  • Must hold a current Medical Assistant Certification.
  • Proficiency with Microsoft Office, general knowledge of patient registration systems and electronic medical records, and basic computer skills to enter, interpret, and extract medical information from electronic systems.
