Key Responsibilities:

  • Prioritize the short and long-term success of our clients by understanding partnership objectives, building trusted relationships with client care teams, providing ongoing feedback on the referral and registration processes, and identifying local opportunities for expansion.
  • Act as a regional liaison between our clients (hospitals and post-acute care partners) and stroke survivors at the beginning of their recovery journey.
  • Assist with registration and enrollment processes, identifying suitable referrals for the Kandu program by collaborating with hospital clinicians to review patient census and triage patients based on their discharge status.
  • Conduct outreach to new Kandu survivor and care partner referrals for program enrollment, which includes educating them about the program, obtaining necessary consent and insurance information, verifying eligibility, scheduling appointments, and providing training on the Kandu app.
  • Support operational tasks at partner facilities to assist mobile app users, including registering new users and addressing access issues, while collaborating with the Product team to resolve complex user scenarios.
  • Reach out to survivors who have paused their services (e.g., due to hospital readmission) to facilitate their return to the Kandu program.
  • Provide technical and customer support to our active app users, both onsite and remotely, ensuring assistance is tailored to the unique needs of stroke survivors.
  • Serve as a liaison to the Kandu Clinical Navigator team regarding survivor impairments and needs that may affect program participation.
  • Communicate access challenges and suggestions for product and process enhancements to Client Services leadership.
  • Prepare regular reports for the Kandu Health team detailing reasons for non-enrollment and recommendations for improving the registration and onboarding experience.
  • Train new Kandu referral clinicians on the registration process.
  • Participate in message testing and identify barriers to registration.
  • Engage in client status calls and meetings, acting as a subject matter expert on the registration process and related data.
  • Support pilot and quality improvement projects at designated client sites as assigned.
  • Attend community outreach meetings to represent Kandu Health and gather new leads from hospital site partners.
  • Daily travel to partner sites is required.
  • Hospital credentialing may be necessary, which could include various health screenings and vaccinations as mandated by client hospitals.

Compensation: Temporary position, 25-29 hours/week - $ /hr. Salary is based on regional standards, with potential for discretionary bonuses.

Required Qualifications:

  • Bachelor's degree in marketing or sales.
  • 4-6 years of direct customer service experience.
  • 2-4 years of experience in a healthcare environment.
  • 2-4 years of marketing or sales experience.
  • Strong communication skills with the ability to adjust messaging for different audiences.
  • Familiarity with health plan benefits and eligibility.
  • Adaptability in dynamic environments to achieve results.
  • Comfortable with technology, including mobile applications and smartphones.
  • Reliable transportation is necessary.
  • Commitment to product knowledge and a willingness to learn.
  • Ability to present information clearly and effectively.

Preferred Qualifications:

  • 2-4 years of experience working with stroke survivors and their families.
  • Conversational Spanish skills are preferred.

Work Environment:

  • Local daily travel to partner hospital sites.
  • Willingness to work late afternoon hours.
