Key Responsibilities:

  • Ensure the long-term success of our clients by understanding their goals and fostering trusted relationships with their care teams. Provide ongoing feedback regarding the referral and registration processes and explore local opportunities for partnership expansion.
  • Act as a regional liaison between our clients (hospitals and post-acute care facilities) and stroke survivors during their initial recovery stages.
  • Assist with registration and enrollment processes, identifying suitable referrals for the Kandu program by collaborating with hospital clinicians to review patient lists and triage patients based on their discharge needs.
  • Conduct outreach to new Kandu survivors and their care partners for program enrollment, which includes educating them about the program, obtaining necessary consents, verifying eligibility, and scheduling appointments.
  • Support operational activities at partner facilities as needed, assisting mobile app users by registering new users and resolving access issues, while collaborating with the Product team to address complex user scenarios.
  • Reach out to survivors who have paused their services to facilitate their return to the Kandu program.
  • Provide support to our community of survivors and care partners, offering technical assistance and guidance on our Kandu App and other technologies, while being mindful of the unique cognitive and emotional needs of stroke survivors.
  • Serve as a liaison to our Kandu Clinical Navigator team regarding survivor needs and concerns that may affect their program participation.
  • Communicate any access issues and suggest improvements to Client Services leadership.
  • Prepare regular reports for the Kandu Health team detailing non-enrollment reasons and recommendations for enhancing the registration and onboarding processes.
  • Train new referral clinicians on the registration procedures.
  • Participate in client status meetings and contribute as a subject matter expert on the registration process.
  • Assist with pilot and quality improvement projects at designated client sites.
  • Represent Kandu Health at community outreach meetings, gathering new leads and reporting back to Client Services leadership.
  • Daily travel to partner sites is a requirement.
  • Hospital credentialing may be necessary, which could include various health screenings and vaccinations as mandated by client hospitals.

Compensation: Temporary position, 25-29 hours per week - $ /hr. Salary includes a region-specific base rate and potential discretionary bonuses.

Kandu Health, Inc. considers factors such as the role's responsibilities, the candidate's experience, education, key skills, and internal equity, as well as market and business considerations when making an offer. This position is eligible for 401k benefits.

Required Qualifications:

  • Bachelor's degree in marketing or sales.
  • 4-6 years of direct customer service experience.
  • 2-4 years of experience in a healthcare environment.
  • 2-4 years of marketing or sales experience.
  • Strong communication skills with the ability to adapt messaging for different audiences.
  • Familiarity with health plan benefits and eligibility.
  • Ability to adjust strategies to achieve results in changing environments.
  • Comfortable with technology, including mobile applications and smartphones.
  • Reliable transportation is required.
  • Commitment to product knowledge and a willingness to learn.
  • Ability to convey information clearly and effectively.

Preferred Qualifications:

  • 2-4 years of experience working with stroke survivors and their families.
  • Conversational Spanish is preferred.

Work Environment:

  • Local daily travel to partner hospital sites.
  • Willingness to work late afternoon hours.
