Job Summary

We are seeking a highly motivated and compassionate Service Coordinator to join our team at SOME, Inc. as a Case Management and Support Specialist. In this role, you will provide critical support to residents, empowering them to overcome daily challenges and achieve personal growth.

Key Responsibilities
  • Provide case management services to residents, including intakes, annual assessments, and referrals to community resources.
  • Develop and implement individualized care plans, utilizing a strength-based approach to support residents' goals and objectives.
  • Assist residents in accessing job training, education, employment, and other supportive services to promote self-sufficiency.
  • Collaborate with the program team to develop and coordinate activities, educational groups, and community events for adults.
  • Identify and present resources and presenters to support activities and events.
  • Document resident progress, incidents, and maintenance repairs, submitting reports as required.
Requirements
  • High School Diploma or equivalent required; 5-8 years of experience in social services, homeless, or substance abuse and mental illness.
  • Knowledge of principles, methods, and procedures for motivational interviewing.
  • Ability to be sensitive to diverse backgrounds and set aside personal opinions and implicit biases.
  • Ability to effectively document resident progress and educate residents on accessing community resources.
  • Ability to perform accurate resident assessments and document progress towards goals.
  • Ability to set professional boundaries.
Working Conditions

This is a safety-sensitive position, and you may be subject to drug testing prior to or during employment. You may be disqualified from employment based on the presence of marijuana in test results, even with a medical card.

This role requires lifting up to 20 pounds, prolonged computer screen use, and occasional off-site meetings.

We offer a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly range for this position is $23.54 to $27.66 and may be commensurate with experience.
