Position Summary:

The Vocational Support Specialist will be an integral part of a collaborative clinical team at Thresholds. This role involves engaging members who are interested in vocational assistance and facilitating job interviews and exploration to promote job retention. Responsibilities include maintaining accurate documentation of individual services and outcomes, such as electronic billing and necessary paperwork.

Key Responsibilities:
  • Ensure confidentiality of consumer records.
  • Perform administrative tasks including answering calls and data entry.
  • Develop, implement, and monitor Individual Service Plans (ISPs).
  • Identify and cultivate suitable employment opportunities in the community tailored to individual needs and preferences.
  • Conduct situational assessments and vocational explorations, documenting consumers' strengths and limitations.
  • Track and report on progress and attendance to align with ISP goals.
  • Exercise sound judgment in administrative decisions impacting service quality.
  • Coordinate with referral sources, community organizations, and service providers.
  • Support consumers in enhancing job-seeking skills, interview preparation, and resume writing.
  • Provide direct support to consumers at their workplaces, offering vocational guidance and fostering essential workplace skills.
  • Prepare and submit timely assessment and progress reports, ensuring all documentation is current and accurate.
  • Maintain consumer files in compliance with HIPAA regulations, safeguarding privacy while adhering to best practices.
  • Engage with consumers, families, and advocates to encourage active participation in program planning.
  • Assist consumers in accessing available resources and benefits, including financial and housing support.
  • Submit monthly attendance and relevant reports to the Program Manager punctually.
  • Provide transportation to worksites as necessary.
  • Participate in staff meetings, training sessions, and professional development opportunities.
  • Perform additional duties as assigned.
Qualifications:

Education:
Bachelor's or Master's degree in social work or a related human services field.

Essential Skills:
Proficiency in Microsoft Word, Excel, and other relevant software.

Physical Requirements:
Ability to lift up to fifty pounds (50).
