About the Role

We're seeking an experienced Registered Nurse to coordinate care for Medicaid and Medicare members in Florida. As a Clinical Coordination Specialist RN, you'll wear multiple hats, working in various environments to ensure seamless transitions and effective health outcomes.

Your responsibilities will include:

  • Providing primary care management for members with complex medical needs
  • Conducting comprehensive needs assessments and developing personalized care plans
  • Partnering with internal teams, providers, and community resources to implement care plans
  • Monitoring compliance and updating care plans as needed

Requirements

To be considered for this role, you'll need:

  • A current, unrestricted RN license in the state of Florida
  • 4+ years of relevant clinical experience
  • 3+ years of experience managing complex populations (e.g., Medicare, Medicaid)
  • 1+ year of community case management experience coordinating care for individuals with complex needs

Preferred qualifications include a bachelor's degree or greater, certification in case management, and experience working with vulnerable subpopulations.
