Job Description

This role involves working as a Clinical Coordinator RN with UnitedHealth Group Inc, providing exceptional care to Medicare and Medicaid members. The successful candidate will wear many hats, interacting with members in various settings, including hospital discharge planning, home visits, and telephonic engagement.

Key Responsibilities

  • Serve as primary care manager for members with complex medical needs
  • Develop and implement person-centered care plans addressing medical, behavioral, functional, cultural, socioeconomic, and social determinants of health needs
  • Collaborate with internal care teams, providers, and community resources to implement care plans
  • Monitor and update care plans, incorporating member feedback and ensuring compliance with interventions

Required Skills and Qualifications

  • Current, unrestricted licensure as a Registered Nurse
  • 4+ years of relevant clinical work experience
  • 3+ years of experience managing complex populations (e.g., Medicare, Medicaid)
  • 1+ year of community case management experience coordinating care for individuals with complex needs
