At Stamford Health, we are seeking a dedicated and compassionate Registered Nurse to join our team as a Healthcare Rehabilitation Specialist. About the Role

As a Healthcare Rehabilitation Specialist, you will play a critical role in delivering patient-centered care that promotes optimal function, health, and independence for individuals with disabilities and chronic illness. You will work collaboratively with physicians, therapy services (PT, OT, ST) and Case Management to develop and implement personalized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Deliver high-quality patient care utilizing the Nursing Process and following established standards of practice within State and Federal guidelines.
  • Supervise non-RN personnel and serve as a resource for ancillary staff on assigned unit.
  • Act as a preceptor to new staff and/or students as requested.
  • Maintain accurate records and reports, including progress notes and incident reports.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Graduate of an accredited School of Nursing.
  • Valid license as Registered Nurse in Connecticut.
  • BLS certification required.
  • Maintain minimum of 6 contact hours annually.
  • Minimum of one year nursing experience; med/surg or rehab nursing experience highly preferred.
