The Role
We are seeking a highly skilled and compassionate Patient Care Coordinator RN - Home Health to join our team. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care services, ensuring seamless transitions between hospital and home settings, and providing education and support to patients and their families.

Responsibilities
+ Coordinate patient admissions, transfers, and discharges efficiently while adhering to hospital policies and regulatory requirements
+ Collaborate with physicians, nurses, and other healthcare providers to identify patient care needs and establish the appropriate level of care
+ Develop and implement individualized plans of treatment in accordance with MD orders, patient care needs, and insurance guidelines
+ Ensure that needed medical equipment, supplies, and ancillary services are ordered and documented
+ Communicate effectively with patients, families, and healthcare providers regarding treatment plans and discharge preparations
+ Delegates specific tasks to support staff, providing clear direction to ensure continuity of care

Requirements
+ Licensed to practice as a registered nurse in the state
+ BSN degree preferred
+ Effective verbal and written communication skills
+ Excellent problem-solving skills and ability to prioritize tasks
+ Ability to work independently and as part of a team
