Job Description

The Patient Services Coordinator II plays a vital role in delivering exceptional patient care at Mount Sinai Health System. As a liaison for patient care activities, this individual ensures quality patient care during patient visits, educates patients on their care plans, and coordinates services with Primary Care Providers and out-patient services diagnostic testing facilities.

Responsibilities
  1. Collaborate with Primary Care Providers to facilitate patients' care plans, participating in daily huddles and monthly case discussions, facilitating appointments, coordinating information, and continuing care requirements.
  2. Work with clinical staff to resolve issues impacting patients' care progression.
  3. Monitor and update dashboards with reminders and alerts to organize daily tasks, such as visit status reminders, follow-up appointments, authorizations for medical review, coordination of specialty appointments, and resolving billing errors.
  4. Contact insurance carriers for patients' pre-certification and consult with Department/Practice Managers regarding patients' needs or addressing escalated issues.
  5. Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
  6. Work as a liaison with Physicians, patients, and clinical staff to connect patients with relevant internal/external resources.
  7. Provide recommendations for process/service improvement to promote patient satisfaction and address issues.
  8. Provide thoughtful and informative responses to patients while guiding them through their healthcare options.
  9. Maintain an understanding of medical terminology and practices used within the hospital through further education.
Qualifications

4+ years of healthcare/customer service or relevant experience.
