We are seeking a skilled Nurse Care Coordinator to join our team at Queens Hospital Center, a state-of-the-art facility with the Queens Cancer Center in southeastern and central Queens. The estimated annual salary for this role is $85,000 - $110,000, based on experience and location.

Job Responsibilities:
  • Assess patients' physical and psycho-social needs to develop and implement comprehensive care plans.
  • Collaborate with healthcare teams to ensure seamless patient care delivery.
  • Communicate effectively with patients, families, and healthcare professionals to promote continuity of care.
  • Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Valid New York State license as a Registered Professional Nurse.
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Graduation from a New York State Education Department-registered nursing education program for unlicensed individuals in Post-Acute Care.
