Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a MDS Coordinator. As a key member of our care management team, you will be responsible for ensuring the timely and accurate completion of the RAI process and care management process from admission to discharge.

With a focus on delivering exceptional patient care, you will work closely with our Director of Nursing and VP of Clinical Reimbursement to coordinate information systems operations and education for the clinical department.

Responsibilities
  • Timely and accurate completion of RAI process and care management process
  • Coordination of information systems operations and education for the clinical department
  • Collaboration with Director of Nursing and VP of Clinical Reimbursement
Requirements
  • Currently licensed as a Registered Nurse (RN) in the state in which you are practicing
  • Prior MDS experience highly preferred
  • Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
