Job Summary

The Referral Coordinator I is a vital member of the Center for Elders Independence team, responsible for providing exceptional patient access services to our clients. This role requires a high degree of accuracy, attention to detail, and excellent communication skills.

Key Responsibilities:
  • Referral Processing: Receive and process referrals in a timely manner, ensuring seamless coordination with participants, caregivers, and the interdisciplinary team.
  • Appointment Scheduling: Collaborate with external specialist/vendor offices to secure appointment dates/times and facilitate the transmission of necessary information.
  • Record Keeping: Maintain accurate records of appointment dates/times and referral tracking in the electronic health record and related systems.
  • Financial Management: Utilize the finance system to obtain referral authorization numbers and enter them into referral and alerts vendor for claims processing.
  • Team Collaboration: Collaborate with the Health Information Coordinator (HIC) to ensure clinical information is available and delivered to external specialists as required.
  • Logistics: Confirm appointments by ensuring all necessary arrangements (transportation, escort, interpreter, etc.) are in place to facilitate successful appointment completion.
  • Communication: Demonstrate effective communication when addressing team member and participant concerns by demonstrating effective problem-solving skills and escalating complex issues to the Health Information and Referrals Manager (HIRM) when necessary.
  • Quality Improvement: Participate in department meetings and contribute to discussions aimed at improving referral processes and service delivery.
  • Dependability: Demonstrate dependability by arriving consistently and on time to scheduled work hours, and submit timely and accurate timesheets.
  • Customer Service: Display a willingness and ability to be responsive to all customer groups, including team members, participants, and their families.
  • Confidentiality: Maintain the confidentiality of all participant information according to HIPAA requirements.
Requirements:
  • Education: Associate degree or equivalent work experience.
  • Experience: Minimum one year's experience authorizing and scheduling medical appointments.
  • Knowledge: Knowledge of medical terminology, completion of medical terminology course preferred.
  • Technical Skills: Experience with electronic health record required, basic proficiency in MS Office systems preferred.
  • Language: Proficient with oral and written English is required.
  • Soft Skills: Customer service oriented with friendly, courteous communication style in person, on the phone, and in writing.
  • Work Style: Detail oriented, high degree of work accuracy, ability to prioritize work according to productivity standards and within deadlines.
