Job Overview

LifeLong Medical Care is seeking a Program Coordinator/Therapist to join our dedicated team. The Program Coordinator/Therapist will manage all facets of the SAMHSA MAT initiative.

This is a full-time, exempt position that offers a comprehensive benefits package.

LifeLong Medical Care is a prominent, multi-site Federally Qualified Health Center (FQHC) committed to delivering innovative healthcare and social services to a diverse patient population. We operate over 15 primary care facilities and provide integrated services, including psychosocial support, referrals, chronic disease management, dental care, health education, home visits, and more. Compensation and Benefits

Salary: $70,000 - $75,000 annually. We provide excellent benefits, including medical, dental, and vision coverage (including for dependents and domestic partners), generous leave policies with ten paid holidays, Flexible Spending Accounts, and a 403(b) retirement savings plan.

COVID-19 Vaccination Policy

In alignment with LifeLong Medical Care's commitment to maintaining a safe workplace, we have implemented a Mandatory COVID-19 Vaccination Policy to protect the health of our employees, patients, and the community. This policy adheres to all applicable laws and is informed by guidance from health authorities. All employees must receive COVID-19 vaccinations unless a reasonable medical or religious exemption is approved.

Key Responsibilities

  • Oversee the integration of mental health, substance use, and MAT services for the SAMHSA MAT initiative.
  • Provide administrative supervision to SAMHSA MAT program personnel, including recruitment, training, performance assessments, and managing personnel matters in accordance with agency policies.
  • Deliver direct substance use disorder (SUD) services, including co-occurring counseling for clients engaged in MAT; outreach services will be provided at various locations, including homeless encampments (approximately 50% of the time).
  • Enhance and support the retention of patients with co-occurring and SUD disorders receiving care from Street Medicine and the Trust Clinic.
  • Collaborate with Trust site leadership and the LifeLong MAT/Recovery Support and Reentry Program Manager to improve coordination of SUD care and services available within Trust Clinic and Street Medicine.
  • Conduct needs assessments and coordinate with community partners to refer patients to suitable external resources.
  • Work with the Evaluator/Research Manager to ensure compliance with all grant reporting requirements.
  • Coordinate care and collaborate with agency-wide MAT and Recovery Support programs to ensure quality and standardized care.
  • Communicate tasks, timelines, objectives, and other essential updates to SAMHSA MAT program staff.
  • Utilize and prepare data in collaboration with the Evaluator to inform decisions impacting the future direction of the SAMHSA MAT initiative.
  • Ensure that grant deliverables are achieved.
  • Gather and provide feedback to program stakeholders in collaboration with the Evaluator.
  • Assess program performance and engage in continuous quality improvement through PDSA cycles.
  • Participate in all required supervision, team meetings, training sessions, regular case conferences, and agency-wide meetings.
  • Perform additional duties as assigned by the supervisor.

Qualifications

  • Must be registered or certified as a SUD Counselor or ASW.
  • Experience in providing substance use and mental health treatment services to diverse, low-income populations.
  • Managerial experience is preferred.
  • Demonstrated ability to work sensitively with a diverse staff and client population, including individuals with mental health and substance use issues.
  • Strong judgment and decision-making skills.
  • Proficient in MS Office applications (including Word and Outlook).
  • Ability to work effectively and calmly under pressure while maintaining a positive demeanor.
  • Excellent interpersonal, verbal, and written communication skills, with the ability to engage with individuals from diverse backgrounds and demonstrate cultural sensitivity.
  • Strong organizational, administrative, and problem-solving abilities, with flexibility to adapt to change.
  • Reliable access to transportation is required.
  • Must comply with all employment conditions set forth by federal and state laws governing healthcare settings and workplace behavior, including strict confidentiality.
  • Experience with Epic Electronic Health Record is preferred.
  • Bilingual proficiency in Spanish and English is preferred.
