CLINICAL NURSE CASE COORDINATOR

I. POSITION SUMMARY: The Clinical Nurse Case Coordinator serves as the primary liaison for the organization, assisting individuals seeking services and support related to HIV/AIDS. This role requires the RN Case Coordinator to deliver exceptional patient care through the formulation, execution, and assessment of personalized patient care strategies. The coordinator will act as a bridge between the organization, the patients, and their families, adhering to established professional care standards, as well as organizational policies and procedures.

This role will begin as part-time with the potential to transition to full-time within the initial months.

III. KEY RESPONSIBILITIES:

A. Ensure that each client enrolled in the Case Management program fulfills the necessary eligibility criteria by performing a comprehensive nursing evaluation of the client's care needs and functional capabilities.

B. Collaborate with the client's healthcare providers, including physicians, social workers, and other relevant professionals to coordinate treatment plans.

C. Maintain accurate records and documentation to report collected data in accordance with state and professional standards.

D. Create a tailored care plan addressing clients' needs, which may include emergency services, nutritional support, healthcare access, HIV education, housing solutions, mental health and substance abuse counseling, and emotional assistance.

E. Identify and address any instances of abuse, neglect, or exploitation that may pose a risk to clients.

F. Empower clients by advocating for their involvement in decision-making and healthcare planning.

G. Track client progress as per their individualized care plan, ensuring regular contact and reassessment of objectives to adapt to their evolving needs.

H. Participate in bi-weekly Client Service meetings and team discussions to consult with supervisors and peers regarding case matters. Engage in annual training sessions to stay informed about services and entitlements.

I. Conduct home visits to provide psychosocial support, assess client needs, monitor progress, and deliver additional supportive services (this requirement has been adjusted due to health considerations).

J. Undertake additional responsibilities as assigned by the Project Director.

IV. CANDIDATE QUALIFICATIONS:

A. EDUCATION: Registered Nurse licensed in California with a degree from an accredited institution.

CERTIFICATIONS: BLS, CNN, BSN required (PHN preferred).

B. EXPERIENCE:

1. Minimum of 2 years of experience in social-service case management and acute care for chronically ill patients, with a solid understanding of the social and economic factors affecting clinical care.

2. Familiarity with the personal aspects of HIV disease and the ability to work sensitively with affected individuals.

3. Strong verbal and written communication skills, coupled with a commitment to follow-through.

4. Experience engaging with diverse communities, including communities of color and the LGBTQ+ population.

C. ADDITIONAL REQUIREMENTS:

1. Bilingual proficiency in Spanish and/or experience working with Hispanic communities is advantageous.

2. Valid California Driver's License and proof of insurance.

3. Proficient in operating a computer and/or tablet.

4. May encounter unsanitary conditions in certain home environments.

5. May be exposed to high-crime areas within the service community.

6. Must be prepared to work in varying weather conditions.

7. Ability to travel within assigned geographic areas as needed.
