Key Responsibilities

  • Oversee program case managers, including recruitment, hiring, onboarding, performance evaluation, and disciplinary actions.
  • Facilitate regular case conferences and team meetings in partnership with the clinical supervisor (LCSW) and community collaborators.
  • Coordinate the collection of program data and generate management reports for monitoring and reporting to funding entities.
  • Establish and nurture relationships with external agencies to enhance program referrals and collaborative efforts.
  • Develop and sustain community resources and connections.
  • Ensure the client referral and intake process aligns with contract and funding objectives.
  • Identify and formulate policies, procedures, and workflows to guarantee contract compliance, enhance efficiencies, promote quality, and achieve program objectives.
  • Organize site tours and shadowing opportunities for partners, funders, and prospective staff to showcase SHP programs.
  • Assess training needs and create/manage staff training initiatives.
  • Prepare and submit reports to funders and engage in grant management activities.
  • Oversee purchasing and equipment management to support case management teams and the SHP administrative office.
  • Champion diversity, equity, inclusion, and belonging within the program framework and in support of SHP participants and staff.
  • Perform additional duties as assigned by the supervisor.

Qualifications

Job Requirements

  • 2-3 years of experience delivering community-based support services for individuals facing homelessness or other special needs, particularly those with mental health and substance use challenges.
  • 1-2 years of experience in program planning and management.
  • 1-2 years of supervisory and management experience within a team setting.
  • Proficiency in MS Suite (including MS Teams) and web meeting platforms.
  • Strong written, verbal communication, and organizational skills.

Job Preferences

  • Bachelor's degree in a relevant field (Public Health, Social Work).
  • Familiarity with Clarity (HMIS), Epic, and/or Community Health Record.
  • Experience with the Coordinated Entry System (CES).
