Position Title: Case Manager - Housing Navigation
Location: Long Beach, CA
Compensation: $28.28 per hour
Employment Type: Full Time
Classification: Non-Exempt

We are looking for passionate individuals to join our Housing Navigation Services team as a Case Manager - Housing Navigation.

ABOUT OUR ORGANIZATION
Since our inception, we have been at the forefront of innovative and effective strategies to support individuals facing homelessness. Our services span across California, connecting clients to a wide range of resources, including homelessness prevention, outreach, employment assistance, personalized case management, and both interim and permanent housing solutions.

TEAM FOCUS
The Recovery Rehousing program is dedicated to assisting individuals experiencing homelessness, particularly those at heightened risk due to health concerns. We provide tailored financial assistance and targeted support services to households, individuals, and youth in need.

ROLE RESPONSIBILITIES
As a member of the Housing Navigation team, the Case Manager will guide clients through various stages of homelessness, including street outreach, interim housing, transitional housing, and permanent housing. Key responsibilities include:

  • Conducting comprehensive intake assessments and collaborating with clients to create Individualized Service Plans (ISPs) that focus on housing objectives, income enhancement, and other personal goals, including health, financial, and social support.
  • Providing referrals and resources to assist clients in achieving their ISP goals.
  • Completing specific program assessments as required.
  • Reviewing client progress and addressing challenges to meet both short-term and long-term objectives.

QUALIFICATIONS

  • Strong analytical skills and attention to detail, with the ability to interpret data and identify trends to inform decision-making.
  • Collaboration skills, fostering positive relationships with colleagues and stakeholders.
  • Effective communication abilities, both verbal and written, to engage and inform diverse audiences.
  • Creativity and initiative, demonstrating a proactive approach to responsibilities and problem-solving.

PREFERRED EXPERIENCE

  • A minimum of two years in healthcare, case management, homeless services, or a related field.

MINIMUM REQUIREMENTS

  • All levels of education and experience are encouraged to apply.

ESSENTIAL REQUIREMENTS
All candidates must:

  • Provide proof of employment eligibility.
  • Be willing to obtain CPR/First Aid certification.
  • Successfully complete a tuberculosis test, background screening, and drug test as part of the hiring process.

DRIVING REQUIREMENTS
Driving is a crucial aspect of this role. Candidates must possess:

  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and vehicle ownership for work-related duties.
  • The ability to meet our organization's insurance criteria.

EMPLOYMENT ELIGIBILITY
Applicants must be authorized to work in the U.S. We do not sponsor employment visas.

WORK LOCATION
All work must be conducted within California. Approval for remote work does not extend to locations outside the state.
