Position Title: Case Manager - Housing Navigation
Location: Long Beach, CA

PATH is looking for dedicated individuals who are passionate about making a positive impact in the lives of others to join our Housing Navigation Services team as a Case Manager - Housing Navigation.

ABOUT PATH
Since 1984, PATH (People Assisting The Homeless) has been at the forefront of innovative and effective strategies to support individuals experiencing homelessness. We provide a wide range of services throughout California, connecting clients to essential resources for homelessness prevention, outreach, employment assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

ABOUT OUR TEAM
The Recovery Rehousing program is dedicated to assisting individuals experiencing homelessness who are at a heightened risk for COVID-19 or have significant needs in securing permanent housing. We offer tailored financial assistance and targeted supportive services to households, individuals, and youth facing homelessness.

ROLE RESPONSIBILITIES
As a member of the Housing Navigation team, the Case Manager will guide clients through various stages of homelessness, including street outreach, interim housing, transitional housing, and permanent housing. Key responsibilities include:

  • Conducting comprehensive intake and individualized needs assessments for all clients, collaborating to create an Individualized Service Plan (ISP) that addresses housing objectives, income maintenance/increase, and other personal goals such as medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Providing referrals, linkages, information, and support to resources that facilitate clients in achieving their ISP goals.
  • Completing program-specific assessments (e.g., VI-SPDAT).
  • Evaluating strengths and challenges in addressing both short-term and long-term goals, conducting a 90-day review with the client post-initial intake.

QUALIFICATIONS
We seek candidates who possess:

  • Analytical Skills and Attention to Detail: Ability to gather and interpret data, identifying trends and insights to guide decision-making while ensuring high-quality work with accuracy.
  • Collaboration and Teamwork: Effective teamwork skills, fostering positive relationships with colleagues, clients, and stakeholders, and contributing to team objectives.
  • Communication Skills: Proficient in conveying information clearly and effectively, both verbally and in writing, to engage and inform diverse audiences.
  • Creativity and Initiative: A proactive approach to work, demonstrating the ability to take action and make decisions independently while generating innovative solutions to challenges.

PREFERRED EXPERIENCE
Two (2) years of experience in healthcare, case management, homeless services, or related fields is preferred.

MINIMUM REQUIREMENTS
All levels of education and experience are welcomed.

MANDATORY CONDITIONS
Regardless of qualifications, candidates must:

  • Provide employment eligibility verification.
  • Obtain CPR/First Aid training.
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING REQUIREMENTS
Driving is an essential function of this position. Candidates must meet the following criteria:

  • Possess reliable transportation.
  • Hold a valid driver's license.
  • Provide proof of insurance and ownership for personal vehicles used during work duties.
  • Qualify for PATH's insurance coverage.
