Job Overview

Position Summary

Under the guidance of the Division Director, Clinical Supervisor, and Program Manager, the Client Care Coordinator will collaborate with a multidisciplinary team to deliver comprehensive services to clients in need.

Key Responsibilities

§ Conduct regular community outreach and educational initiatives.

§ Collaborate with Adult Protective Services to mitigate immediate risks faced by clients.

§ Assess client eligibility for services through in-home evaluations and facilitate service arrangements and authorizations.

§ Create personalized care plans that ensure a culturally sensitive and language-appropriate system of services.

§ Maintain detailed and accurate documentation of client interactions and services provided.

§ Explore suitable options for both short- and long-term care, empowering clients and their families to maintain independence and self-determination.

§ Establish and nurture a support network for clients, including family and community resources, through ongoing case monitoring.

§ Build and sustain relationships with both public and private service organizations, ensuring a diverse and inclusive approach.

§ Manage a client caseload with a maximum ratio of 40:1.

§ Collaborate with a multidisciplinary mental health team to provide 24/7 crisis intervention services.

§ Availability for evening and weekend hours as required.

§ Participate in monthly Care Plan Team Conferences and other group supervision activities.

§ Responsible for all monthly documentation, data collection, and billing related to client caseload.

§ Advocate for issues impacting the population served.

§ Travel as necessary for meetings, client visits, and other relevant events.

§ Perform additional duties as assigned by the Division Manager and Program Coordinator.
